Online PHDS Family Tool Privacy Notice

Purpose

This Online Promoting Healthy Development Survey (“Online PHDS”) website was developed and is maintained by the Center for the Advancement of Innovative Health Practices doing business as the Child and Adolescent Health Measurement Initiative (“CAHMI” or “we” or “us”). This Privacy Notice describes the information we process in connection with your use of the Online PHDS.

CAHMI is governed by the Health Insurance Portability and Accountability Act (HIPAA) (42 U.S.C. §1301 et seq.). Such use of your and your child’s personal information shall be protected in compliance with HIPAA.

How information is used

CAHMI recognizes that your personal data belongs to you. We are committed to giving you control and genuine choice about how your data is used. We strive to be transparent about our data usage and provide you with the information you need to make informed choices.

The information gathered on the Online PHDS is used by three groups of people:

  1. Parents/Caregivers and families (collectively “you”) who fill out the Online PHDS receive a personalized report based on their responses. This report helps guide parents/caregivers and families about the steps they can take to partner with their child’s healthcare providers to improve their child’s health care experience.
  2. Providers, practices and organizations (collectively “Providers”) who have an account with the Online PHDS can generate an aggregated summary report of the findings, specific to their practice or organization.
    • This report presents information to Providers that can be used to improve care to young children.
    • Providers can register to customize the Online PHDS specific to their practice or organization and focused on the specific areas of health care they want to improve.
    • At the beginning of the survey, parents/caregivers are asked to identify where their child receives care. If a parent/caregiver receives a link to this survey from a Provider, it means the Provider has created an account with the Online PHDS and the Provider can generate a report summarizing aggregated responses to the Online PHDS. Providers will not know what each parent/caregiver’s individual answers are or be able to identify who completed the survey.
  3. CAHMI may use the de-identified aggregate findings from the Online PHDS for research purposes.
    Additional examples of how your information and data may be used includes:
    • User Experience: We are dedicated to optimizing your data for your experience and use. We use information you share to provide resources, offer customizations, and improve your experience with the Online PHDS.
    • Tool Support and Improvement: We use your data to drive improvements, resulting in better performance and enhancements. This data helps us identify issues and make necessary updates to improve the Online PHDS for our users.
    • Research & Innovation: When you choose to share feedback with CAHMI on the Online PHDS, we use your data to understand what Online PHDS users want and guide our development efforts. This data allows us to detect usage trends and gain real-world feedback on what features and services our users need.

Managing Your Personal Data

The Online PHDS does not collect protected health information or information that can lead to the identification of you or your child. The Online PHDS requests one piece of personally identifiable information: date of birth. We do not store or retain this data. The date of birth is used to calculate the appropriate most recent well-visit for your child and present age-specific questions. The age visit (e.g. 4 month or 6 month) is stored in our database rather than the date of birth. In addition, identifying information connected to your computer (IP address) will not be recorded by CAHMI at any time.

How your information is protected and secured

Data Minimization and Retention:

We will keep personal data for no longer than is necessary for the purposes for which it was collected and then we will securely delete or destroy it.

Data Security

We implement information security best practices and controls to keep your personal information safe. Both security and privacy are embedded throughout our design and developmental processes. To protect your personal information and data, we implement a variety of security measures including: use of secure server, information storage on secure dedicated port-protected database that only allows data entry from the tool to our database, database with closed-off port for the SQL Server installation. And, we follow the principle of least privilege access to ensure that your personal data can only be accessed on a need-to-know basis. Only authorized personnel with special access rights to our systems are given access to the data.
