Go to the Online Promoting Healthy Development Survey public website (PHDS) (www.onlinephds.org) and:
- Type in the CODE you were given to use (short name like “DrJones”)
- Complete the Online PHDS and email your Online PHDS Feedback Report to your child’s provider on your own (or bring it with you to the visit).
